Submission of this application form for a Gaming Distributor Licence will allow a Regular employee to obtain a licence to operate within a licensed betting or gaming establishment.

The betting or gaming establishment shall submit this Form to the Gambling Control Commission, upon receipt a notification letter will be sent to request payment in the amount of $——- which is a non-refundable processing fee.

Only requests with correct, complete, and documentary information requirements will be processed. Applications with incorrect and/or incomplete information will not be processed and the licensed establishment will be notified of the Commission’s decision.

1. COMPLETE THIS FORM:

A. This application form is to be completed by any person who wishes to apply for a Gaming Distributor Licence, directed to do so by the Commission.
B. At the direction of the Commission, applicants submitting this form complete and submit the Personal History Disclosure Form and Business History Disclosure Form.

2. ESTABLISHING YOUR NATIONALITY AND WORK AUTHORISATION:

A. All applicants will be required to verify their identity and employment authorisation, at the request of the Commission the verification process includes the submission of original documentation to the following address:

3. IMPORTANT NOTICES:

A. Should you be unable to fully understand this form in English, you must acquire adequate means of translation.

B. All notices regarding your application will be sent to the email address and postal address that you provide on this form. You must immediately notify the Commission of any changes during the operation of your entity.

C. Pursuant to Section 46(c) of the Gambling (Gaming and Betting) Control Commission Act No.8 of 2021, failure to comply with answering any of the questions completely and truthfully will result in the revocation of your application.

D. You are hereby applying for a Gaming Distributor Licence as referred to in Section 30(2)(j) of the Gambling (Gaming and Betting) Control Act No.8 of 2021 for reasons stated in this application form, pertaining to the Casino licence that is (to be) granted for the operation of the casino.

E. A licenced gaming machine distributor is required to display (i.e., wear in a clearly visible manner) the approved technician ID while servicing, repairing, or maintaining approved gaming machines in a club or hotel.

F. Pursuant to Section 32(4) of the Gambling (Gaming and Betting) Control Commission Act No.8 of 2021,

“An applicant under this section shall, prior to the application being accepted, consent to the use by the Commission of the personal information contained in the application-

(a) for the purpose of processing the application; and
(b) where a licence is granted, for the maintenance of the Register, with Section 51, and such other purposes of this Act as the Commission thinks fit, and that personal information shall remain confidential in accordance with section 88.”

Excluded as references are blood relations and relatives to the third degree, the life partner (including the spouse) and former life partner of the applicant, persons subject to a professional obligation of secrecy, and persons who are financially dependent on the applicant.
