Submission of this application form for a Betting Operator’s Licence will allow a licensee to operate a betting shop for the purpose of conducting betting.

The establishment must complete and submit this form to the Gambling Control Commission. Upon receipt of the application, a notification will be sent to the applicant requesting payment of the non-refundable Application Processing Fee. Other fees are outlined in Section 3.

Only applications that are fully completed and meet all applicable requirements will be processed.
